Attestation

You must do an emergency preparedness attestation each year if your service:

Important:

  • You only need to attest once per year even if you run more than one service.
  • You must complete the Emergency Preparedness Attestation form by 30 November each year.
  • The form is available on the Funded Agency Channel.

When to do your attestation

Fill out the attestation form online before the high-risk emergency season begins. 

This helps us:

  • understand how ready services are for emergencies
  • report to the Emergency Management Commissioner
  • improve emergency management policies and resources.

How to complete your attestation

  1. Go to the Funded Agency Channel.
  2. Log into My Agency.
  3. Complete the Emergency Preparedness Attestation form in the Service Agreement Module (SAM).

People with disability

The Person-Centred Emergency Preparedness (P-CEP) toolkit helps services that work with people with disability to create tailored emergency plans.

The University of Sydney developed the toolkit in collaboration with:

  • people with disability
  • support services
  • government agencies
  • emergency personnel.

General resources

The Australian Red Cross' RediPlan has easy-to-follow actions to help people plan for emergencies. Get Prepared is a free, easy to use app that helps you find information and tools to do an emergency plan. It builds on the Red Cross RediPlan.
