Attestation
You must do an emergency preparedness attestation each year if your service:
- gets funding from the department, and
- is covered by the Social services sector emergency management policy. View the Social services sector emergency management policy (Word 345 KB).
Important:
- You only need to attest once per year even if you run more than one service.
- You must complete the Emergency Preparedness Attestation form by 30 November each year.
- The form is available on the Funded Agency Channel.
When to do your attestation
Fill out the attestation form online before the high-risk emergency season begins.
This helps us:
- understand how ready services are for emergencies
- report to the Emergency Management Commissioner
- improve emergency management policies and resources.
How to complete your attestation
- Go to the Funded Agency Channel.
- Log into My Agency.
- Complete the Emergency Preparedness Attestation form in the Service Agreement Module (SAM).

The Person-Centred Emergency Preparedness (P-CEP) toolkit helps services that work with people with disability to create tailored emergency plans.
The University of Sydney developed the toolkit in collaboration with:
- people with disability
- support services
- government agencies
- emergency personnel.
General resources
The Australian Red Cross' RediPlan has easy-to-follow actions to help people plan for emergencies. Get Prepared is a free, easy to use app that helps you find information and tools to do an emergency plan. It builds on the Red Cross RediPlan.
