Statutory Declaration – Pre-employment screening for disability workers (DWES)

Registered disability service providers may use this document to assist them with meeting the requirements of the Disability Worker Exclusion Scheme. This Statutory Declaration should be completed and declared by a worker prior to being engaged by a disability service provider and complements all other safety screening requirements for workers.
